This sales agreement can be used for gas, oil and electricity service.

In field one, insert the name as it appears on your utility bill
In field two, insert the address as it appears on the utility bill
In field three, insert the account number as it appears on the utility bill
In field four, insert the utility's name, i.e. KeySpan, Con Ed, etc.
In field five, insert the first of the month that service should begin
Field six is self-explanatory
Field seven is optional (for customers who choose budget billing)
Sign and date the fields labeled customer and buyer
Return the completed form to your local union representative or by fax to (631) 979-5281.

Once the completed sales agreement is processed, Columbia Utilities will mail a copy to you.
